 If you want to find out if your "WITH PROFITS" endowment policy is sufficiently valuable for an endowment buyer to make on offer, complete the short form below. Do not complete the form if your policy is "unit linked" as these policies are not tradable.

  Maturity Date What's This write as above, using forward slash (/)
  Gross Premium What's This Do not use £ sign (write £51.92 as 51.92 using full stop to separate pounds and pence)
  Premium Frequency What's This (usually monthly)
  Basic Sum Assured What's This Do not use commas or £ sign (write £10,234 as 10234 omitting the comma and £ sign)
  Total Bonuses What's This Do not use commas or £ sign
  Bonus Year What's This write as 2010 for example
  Surrender Value What's This  Do not use commas or £ sign (write £23,567.90 as 23567.90)
  Surrender Value Date What's This This must be no more than 5 days old or the policy cannot be valued. Write same style as dates above using forward slash (/)

Additional Vital Information

Include here 1st applicants date of birth (DD/MM/YYYY)

2nd applicants date of birth (DD/MM/YYYY)

Details of any policy alterations i.e. assigned to one applicant only.

Finally state if the policy was a "low start" policy (premiums start off low and then increase in stages over the first 5 or 10 years)
